Beyond the Monofilament for the Insensate Diabetic Foot

چکیده
Foot ulcers in patients with diabetes lead to infections, amputations, and high costs, and their prevention is a stated goal of the American Diabetes Association. To assess the benefits of various interventions on the prevention of future diabetic foot ulcers, we searched for and reviewed all randomized clinical trials (RCTs) on the prevention of diabetic foot ulcers and evaluated their efficacy and scientific validity on the basis of an established systemic grading system. Only 13 RCTs were identified. All involved secondary prevention or a mixture of primary and secondary prevention. Most were small and of poor quality, with negative studies generally being of better quality than positive studies. Of all methods proposed to prevent diabetic foot ulcers, only foot temperature-guided avoidance therapy was found beneficial in RCTs, although this needs to be validated in other populations. These observations only apply to high-risk populations, and the benefits to the general population with diabetes are unclear.

A comparison of the monofilament with other testing modalities for foot ulcer susceptibility.
We studied the number of testing sites and the proportion needed to be insensate for the optimal assessment of foot ulcer risk with the 10 g monofilament. Also, we compared the sensitivity and specificity of the 10 g monofilament with other methodologies. Fifty-two individuals with either a current foot ulcer, a history of a foot ulcer or the presence of Charcot neuroarthopathy and 51 individua...
متن کاملImproved sensitivity in patients with peripheral neuropathy: effects of monochromatic infrared photo energy.
The medical records of 1,047 patients (mean age, 73 years) with established peripheral neuropathy were examined to determine whether treatment with monochromatic infrared photo energy was associated with increased foot sensitivity to the 5.07 Semmes-Weinstein monofilament. The peripheral neuropathy in 790 of these patients (75%) was due to diabetes mellitus. Before treatment with monochromatic ...
متن کاملThe Ipswich Touch Test: a simple and novel method to screen patients with diabetes at home for increased risk of foot ulceration.
AIMS The Ipswich Touch Test is a novel method to detect subjects with diabetes with loss of foot sensation and is simple, safe, quick, and easy to perform and teach. This study determines whether it can be used by relatives and/or carers to detect reduced foot sensation in the setting of the patient's home. METHODS The test involves lightly and briefly (1-2 s) touching the tips of the first, ...

متن کاملA Review of Total Contact Casts for Offloading Diabetic Foot Ulcers
Dr Paul Brand was the first to widely use Total Contact Casting (TCC) in the mid 1960’s to offload the insensate foot in Hansen’s disease (Sinacore et al, 1987). It has since been identified as a ‘Gold Standard’ for offloading diabetic foot ulceration within the diabetic foot-care community (Boulton, 2004).
